At Glory Hospice, we understand that receiving a terminal diagnosis can be frightening and overwhelming for both patients and their loved ones. For this reason, each patient is assigned a hospice medical team. The team makes home visits to provide reassurance and the support needed for peace, comfort, pain, and symptom management.
The hospice team consists of:
· Physicians and Nurse Practitioners
· Nurses with over 40 years of experience
· Licensed Medical Social Workers
· Chaplains for spiritual connection
· Hospice Aides for personal care (i.e, bathing)
· Homemaker and Volunteers
· Dietician and Physical Therapist if needed
· Primary Physician Involvement
. 24/7 Nurse Triage Call Center
· Medical Equipment (i.e, hospital bed, oxygen, wheelchair
· Medical Supplies (i.e, briefs, wipes, gloves, chux)
· Wound Care Supplies
· Medications Related to Diagnosis
· Volunteer Services
· Ancillary Services (i.e, dietary consult)

Levels of Care:
· Routine - Care in the comfort of your home
· Continuous Care - Management of uncontrolled symptoms
· Respite Care - Short-term relief for your caregiver
· Inpatient - Acute setting to manage symptoms
